What is CAIRE?
Comprehensive Adverse Incident Reporting & Evaluation (CAIRE) is an incident reporting tool that leverages incident data to help take appropriate measures in preventing such occurrences from happening again in the future.
How will the provided Information be used?
By default, your submitted event report will be included in our anonymized, aggregated reports. In submitting the report you will be given the option to allow us to use the information you provide in an (anonymized) case study, or to submit a copy of the information to state, federal, or accreditation organizations. CAIRE will not release your identifying information to anyone without your consent.
Is it safe to Share information?
All personal identifying information shared with the CAIRE portal is confidential and will only be accessible by the CAIRE Review Panel. The individual submitting the information has full control over how the information is utilized. The CAIRE Review Panel has an attorney on staff which qualifies the information as protected under attorney/client privilege rules should any retaliatory or whistleblower concerns exist.
Who will Review this information?
The CAIRE Review Panel is made up of safety certified professionals within your discipline. They are nominated and elected to 2-year terms of service and are subject to the confidentiality limitations established by the individual submitting the information. The CAIRE Review Panel is represented both geographically and size / type of practice –creating a peer reviewed environment for incident analysis.

Your submittal will provide the critical data points necessary to accurately analyze safety within your profession. If elected by the individual submitter a case study of the event along with root cause analysis and recommended improvement measures will be generated by the CAIRE Review Panel.

How to CAIRE?

Witness Incident

CAIRE wants information on MRI incident which did or could have caused harm to anyone. If you have concerns, submit your report to be on the safe side.

File a Report

Please provide all necessary details about the incident, including what happened, who witnessed it, where it happened, and why it happened. If you don't have all the information, just leave it blank.
